It is well known that negative events (costly operational disturbances, incidents, accidents) sometimes occur despite information regarding the key vulnerabilities being available to the organisations involved. At the extreme, this causes major business disturbances or so called man-made-disasters, through the unaddressed latent contextual factors. Some of those latent factors can be traced back to vulnerabilities, or ‘blind spots’, in systematic safety work. To identify the blind spots in the Swedish Air Navigation Services (LFV) safety work, a workshop methodology was devised. In the presentation, the workshop methodology, results, and lessons learned are discussed.

Dr Lundberg is a senior lecturer in Information Design at Linköping University, Sweden. His research has addressed methods for systematic safety work, and design of interactive information environments. This includes research on design of interactive visualizations, as well as analysis frameworks to understand safety and resilience of human activity in high stakes domains.
